I saw this fish while diving the south side of Grand Cayman last week, and found it rather bizarre. It didn't want to be photographed, and a blue chromis got in the way, but it's what appears to be a white grunt with a large dusky body stripe behind the pectoral fin, and a couple of blotches on the tail. It was at approximately 15-20 feet.
Has anyone seen anything like this or know what causes it?
White Grunt
Yes, it's a White Grunt, Haemulon plumieri. The dusky bar is normal, your grunt just is a bit more vivid than most. You'll notice that dusky area is often more prominent in juveniles.

Thanks for the clarification. I knew they had a single large tail blotch as juveniles, but I wasn't aware they had dark body stripes as well.
