Thank you for joining us on a REEF Field Survey Trip! Each participant on a trip should complete and submit this form. Please contact trips@REEF.org with any questions.

New to REEF Trips and wondering what those acronyms like TWA, CIP, TEP and PAC mean? For more information on REEF's survey regions, check out our Volunteer Fish Survey Project

Some trips will have separate forms and are not listed above. If you are attending one of these trips, you will receive a different link for your form.
If you aren't yet a member, please join online. If you don't know your number, we can look it up for you.
All REEF Trip participants may choose to receive a survey slate or a starter kit, which contains everything you need to begin surveying. If you have never surveyed before, we highly suggest selecting the starter kit option. We have two types of slates - the original yellow slate for TWA, EAM, TEP trips and the larger white slate for CIP, SOP, IORS trips.
There are two types of paper for trips located in the Tropical Western Atlantic (TWA) survey region. "Short form" paper lists the most frequently sighted species and also has plenty of space for write-ins and sketches. "Long form" paper lists more species and has less space for write-ins and sketches. Experienced surveyors may prefer the long paper. Both paper types fit the yellow survey slate. If no paper preference is indicated, you will receive short paper.
All participants will be mailed some REEF material before the trip, including a BCD tag and a region-specific waterproof color ID card. If you do not wish to receive some or all of these items, please indicate that in this space. If this field is left blank, you will receive a BCD tag and color ID card along with your underwater survey paper and a survey slate or starter kit (if requested above.) Note: if you select the "starter kit" option above, you will automatically receive a BCD tag and color ID card as these materials are part of the starter kit.
