The purpose is to gather information from participants on REEF Field Survey Trips and other REEF projects so that we can best serve their needs. In addition to general information on your previous experience with REEF projects, roommate preferences, and other information, we are also asking that all participants in REEF programs report their COVID-19 vaccination status.

In order to ensure the health and safety of REEF members and staff, as well as for the dive and hotel staff we are working with and the residents of the surrounding communities, REEF is asking that all participants in REEF Field Survey Trips, Advanced Assessment Team Projects, and other special projects be fully vaccinated for COVID-19 prior to participation (if eligible) or comply to additional safety measures**. For purposes of this form, you are considered “fully vaccinated” two weeks after completing the second dose of a two-dose COVID-19 vaccine (e.g., Pfizer or Moderna) or two weeks after receiving a single dose of a one-dose vaccine (e.g., Johnson & Johnson). You can complete this form multiple times to provide new information on COVID boosters or other details.
